**Cigna Foundation-WORLD OF DIFFERENCE GRANTS IN COMMUNITY HEALTH NAVIGATION – Individuals can’t achieve their best possible health if they don’t understand the health challenges they face. They can’t access community health resources if they don’t know they exist.
